  • Is a physician’s referral/order required to attend physical therapy?

    No. Illinois is now a direct access state and does not required a referral/order.

  • What should I wear to my appointment

    No special clothing is required. Patient gowns are provided.

  • What should I expect during my initial evaluation?

    A thorough verbal history is taken, followed by a physical exam, including a pelvic exam to assess the function and condition of the perineal and pelvic floor anatomy. An external examination of the surrounding muscles and tissues will also be assessed.

  • How often do I need to be seen?

    Typically, patients are seen once a week for 1 hour. Exceptions are made to accommodate a patient’s schedule (for example, returning college students).

  • How many visits are required?

    In our experience, we allow the patient to complete 10 visits to determine if they are benefiting from our treatments.

  • When should I start seeing improvements?

    Usually, changes are noted within the allotted 10 visits. Improvement and changes are dependent on the duration of symptoms and the patient’s compliance to their therapist’s recommendations.

  • Can I come to pelvic physical therapy on my period?

    It is the patient’s choice. Treatment can be modified for the patient’s comfort.

  • What are your fees?

    In order to provide you with the highest level of care at the lowest possible prices, we require payment in full at the time of your treatments.

    Each one hour appointment is $180. You will be provided with 2 copies of your paid receipt which includes the necessary information needed to complete and submit an insurance claim. We are NOT part of any insurance network. Therefore, Kotarinos Physical Therapy, LTD. will be considered an Out-of-Network provider with ALL insurance companies.


    We do NOT treat Medicaid, Workers' Compensation cases or cases involving lawsuits. We are NOT a medicare provider. If you have Medicare, please contact the office for additional information.


    All major credit cards, including FSA and HSA cards accepted.


    We strongly encourage you to contact your insurance company to verify your out of network physical therapy benefits. Many health insurance companies have contracted with Utilization Management companies to determine physical therapy coverage benefits. Be sure to inquire if your insurance company in one of these and how it affects your reimbursement. Below is a list of information you may be asked to provide when verifying your coverage.

    CPT Codes

    Here is a list of our frequently used CPT codes. The treatment codes are billed per unit (1 unit = 15 minutes). A 1 hour appointment will be billed as 4 units of treatment codes. Your therapist will choose the codes based on the treatment provided to you during that day's treatment session.
